This patient had a severe anterior two-plane open bite. Therefore, extrusion of the anterior teeth and intrusion of the posterior teeth were important. Proper arch expansion was performed simultaneously. To intrude the molars, ghost attachments were designed on the occlusal surfaces of the molars. Additionally, the patient was instructed to perform soft clenching exercises daily. For the anterior teeth, optimized multi-tooth extrusion attachments were designed. Through molar intrusion, counterclockwise rotation of the mandible was achieved, and vertical facial height was also reduced. To improve the Bolton ratio, IPR was performed on anterior teeth. This is a good treatment case demonstrating that open bite can be successfully treated with Invisalign treatment.
