What learning would you share with other Invisalign providers?
Placing attachments on both the buccal and lingual surfaces of the molar adjacent to an extraction site provides better control over crown-root angulation and helps prevent tipping during space closure. In growing patients with partially erupted teeth, appropriate extraction timing combined with eruption compensation allows natural eruption to contribute to space closure, reducing the amount of active tooth movement required from the aligners.
What contributed most to this treatment being successful?
In the lower arch, attachments were placed on both the buccal and lingual surfaces of #36 and #46 to maintain control during space closure following extraction of #35 and #45, preventing mesial tipping of the molars. In the upper arch, #13 and #15 had not fully erupted at the start of treatment, so eruption compensation was applied. With extraction timed appropriately and Invisalign aligner intervention started at the right stage, the unerupted teeth erupted naturally into the extraction site, and the #14 space was closed to a favorable degree by the end of the primary aligner order.
