Summary

The patient presented with severe Class II Division 1 malocclusion with deep bite and overjet. Tooth 35 is linguoverted and presented with a Scissors bite relationship with tooth 25. The treatment plan consists of anterior teeth IPR, expansion and maxillary molar distalization, accompanied by retraction and intrusion of maxillary anterior teeth. Distalization of the left mandibular molars with TADs anchorage is performed to create space for uprighting and aligning tooth 35. Mandibular anterior teeth will be intruded in stages, and Class II elastics will be utilized in the later stage of treatment. As this was a low-angle patient, mild molar extrusion was planned to assist the correction of deep bite. After completing 65 Invisalign clear aligners, the deep bite and overjet were both corrected. The molar relationship was converted from Class II to Class I, and the patient’s profile improved significantly. No additional aligners were used — a single series of aligners successfully treated this severe skeletal Class II case. After 34 months of treatment, the patient entered the retention phase. Follow-up photographs taken 42 months post-treatment showed well-maintained occlusion with stable cusp-fossa relationships, and the patient was highly satisfied with the outcome.
