Summary

This extraction case is quite interesting, as the Invisalign G6 protocol was applied to almost both the upper and lower arches. The upper arch presented a fairly standard G6 setup, while the lower arch exhibited a significant amount of crowding. Consequently, after utilizing the extraction space for crowding resolution, there was minimal residual space left to close. This actually lowered the overall biomechanical difficulty. As a result, the G6 protocol performed exceptionally well in this case. However, a specific issue emerged during the Additional Aligner (AA) phase: although the crowding was successfully resolved, the lower anterior teeth were relatively retroclined. Because the posterior mechanics relied on the G6 default setup, it essentially lacked the ability to effectively control the anterior root angulation. Therefore, we focused significant effort during the AA phase on adding lingual root torque to the lower anteriors. This was done to improve the aesthetic outcome and upright the incisors to an inclination of approximately 90 degrees (IMPA ~90°). This torque adjustment was the primary objective of the first set of AAs. Ultimately, this case is quite fascinating, especially since my current treatment design approach might sometimes differ. It indicates that the G6 protocol remains highly effective in such cases, provided that a deep Curve of Spee is avoided in the setup.
