Patient Chief Concern
The patient presented with a deep bite and a Class II molar relationship, seeking functional improvement and esthetic correction.
What learning would you share with other Invisalign providers?
Deep bite correction in extraction cases can be particularly challenging with conventional fixed appliances. The vertical force of interarch elastics in these scenarios often creates a "bowing effect," exacerbating the deep bite rather than correcting it. Such cases often require temporary anchorage devices (TADs) for vertical control.
the Invisalign system offers a distinct advantage by providing precise, programmed incisor intrusion.
In this case, precision bite ramps could not be utilized, so conventional bite ramps were placed on the upper canines to assist with disarticulation. This hybrid approach, combined with aligner mechanics and elastics, successfully corrected the deep bite without the need for TADs.
What contributed most to this treatment being successful?
The treatment plan included extraction of the maxillary first premolars, interarch elastics, and interproximal reduction (IPR) to coordinate dental midlines. Excellent patient compliance with elastic wear played a critical role. Invisalign aligners provided effective vertical control throughout treatment, resulting in successful deep bite correction and stable occlusal outcomes.
