The third molar can be orthodontically protracted to replace the second molar, and the second molar can be moved into the first molar space, achieving a prosthesis-free outcome.

In the mandible, sequential distalization was performed using TADs. In the maxilla, bilateral first molar spaces were closed using a Type B anchorage strategy. A lever arm and an elastomeric chain were applied to prevent tipping of the teeth adjacent to the missing space.

Summary

A stable Class I canine and molar occlusion with coincident dental midlines was obtained. The bilateral 7mm maxillary first molar spaces were completely closed with ideal root parallelism. The maxillary second molars successfully replaced the first molars, and the third molars migrated into the second molar positions.
