Invisalign First

Patient Chief Concern
Large overjet, Skeletal Class II

What learning would you share with other Invisalign providers?
I initiated treatment with pre-MA. Due to the presence of multiple missing teeth and insufficient aligner retention, pre-MA was essential for this patient. Following the pre-MA phase, I proceeded with MA. In my clinical protocol, I instruct the patient to wear each MA stage for two weeks. This approach is designed to achieve sufficient skeletal effects by allowing adequate time for mandibular growth stimulation.

What contributed most to this treatment being successful?
I based the treatment on a precise understanding of both pre-MA and MA mechanics. In my opinion, aligner retention is the key factor for success during the MA phase. The decision to separate the pre-MA and MA phases was the most critical strategic element in this patient’s treatment.

Summary

Mandibular Advancement (MA) is an excellent treatment modality. In this case, I utilized MA during the patient’s peak growth period to establish ideal overjet (OJ) and overbite (OB). Although the patient is currently awaiting the second phase of treatment, the occlusion has been corrected so effectively that a second phase may barely be necessary. This case clearly demonstrates the therapeutic potential of MA.
