Patient Chief Concern
Protruded and crowded teeth
What learning would you share with other Invisalign providers?
In cases of severe overjet, extraction of upper premolars can be a treatment option. To make root parallel between extraction site, proper gable bend during space closure is needed. Patient compliance is strongly needed to make this goal.
What contributed most to this treatment being successful?
Patient showed severe protrusion of upper anterior teeth, and large overjet. Mandibular crowding was less crowded and protruded so decision was made to extract the upper first premolars only and make Class II molar relationship. To prevent lingual inclination of upper anterior teeth during space closure, lingual root torques on upper anteriors were placed 7 degrees. To prevent tipping between extraction site, 15-20 degrees gable bends were placed on adjacent teeth and place 4-5 degrees of anti-tipping bends on adjacent teeth near extraction sites (from the beginning to the middle stage). To correct occlusal seating and make stable overjet and overbite, Class III elastics was used in the Additional Aligner stage.
