The aligner is a highly effective tool for maxillary expansion and arch alignment. To secure eruption space for permanent teeth, overcorrection in arch expansion can be planned. As the erupting canine is larger than the primary canine, additional mesiodistal space can be allocated in the ClinCheck final stage.
What contributed most to this treatment being successful?
The patient exhibited a significant lack of eruption space for permanent teeth in both the maxilla and mandible. The mandibular dentition was generally shifted toward the left, resulting in severe midline discrepancy, and there was insufficient eruption space for the left mandibular canine. Additionally, the maxillary lateral incisors were rotated to the extent that they aligned in a straight line with the central incisors when viewed occlusally. Teeth 1.4 and 2.4 also had inadequate eruption space, causing them to erupt buccally. To secure sufficient eruption space, extensive arch expansion was planned for both the maxilla and mandible, with expansion overcorrection. The left-shifted mandibular dentition was guided toward the right to achieve maximum midline coincidence. Distal rotation values were set for the lateral incisors to achieve a more natural alignment (1.2: 13.5D, 2.2: 22.4D). Furthermore, in the final stage, the posterior occlusion was intentionally overcorrected and set with strong contacts.
