Invisalign First

Patient Chief Concern
Protrusion of the teeth

What learning would you share with other Invisalign providers?
I would like to share how to determine the appropriate amount of expansion and create a ClinCheck plan for pediatric patients with narrow arches and protrusion, in order to improve aesthetics and support the normal eruption of permanent teeth.

What contributed most to this treatment being successful?
It is the expansion capability and sequential distalization of the Invisalign system.

Clinical Indications

  • Class II div 1
  • Crowding
  • Increased overjet
  • Maxillary midline deviation
  • Midline shift
  • Narrow arches
  • Rotations
The bilateral first molars show a Class II relationship, which has resulted in crowding and protrusion of the maxillary anterior teeth. Most notably, the arch was asymmetrically distorted on both sides.

Modalities

  • Distalization
  • Elastics: Class II
  • Intrusion
  • Leveling Curve of Spee
  • Overtreatment
  • Phase I treatment / Interceptive
The sequential posterior movement of the teeth was maximized using elastics, allowing for the correction of anterior protrusion, arch expansion, and securing space for permanent teeth. During this process, an open-bite configuration overtreatment was designed in the ClinCheck to prevent the bite from deepening while improving the anterior teeth.

Treatment Information

Retainer: Vivera retainer
For Invisalign First patients, Vivera retainers become the first choice to maintain the alignment of the anterior teeth and provide a structure that accurately accommodates the eruption of permanent teeth in the spaces left by exfoliated primary teeth.
Submission Process: iTero Scanner Element/5D
I believe the iTero 5D scanner is an excellent tool as it enables accurate scanning for Invisalign treatment and allows for the evaluation of the treatment process.

Summary

Posterior movement and arch expansion of the maxillary teeth improved the protrusion of the maxillary anterior teeth and reduced the overjet. Intrusion of the mandibular anterior teeth stabilized the overbite between the upper and lower arches. Additionally, sufficient space was secured for the eruption of permanent teeth, resulting in a stable occlusion.
